1. Life Insurance Plan Options

    What is the right plan for me?

    Generali Pilipinas offers you a wide range of peso and dollar-    denominated life insurance products to create a secure future for you     and your family.

    Here is a list of the most common financial needs people have, which     life insurance can meet. Even before choosing a product, you will first     have to decide which needs represent your highest priority.

    • Financial security and income replacement in case the family       breadwinner dies
    • Funds for final expenses, such as hospitalization and funeral costs,       upon the death of the family breadwinner
    • Funds for debt liquidation upon the death of the family breadwinner
    • Funds for your children’s education
    • Savings for your personal needs and aspirations

    • Protection and financial security for your employees

    I am not sure what kind of life insurance plan is most beneficial to me     and my family. Can someone help me make a decision?

2. Choosing My Beneficiaries

    Who can I assign as my beneficiaries?

    The most common beneficiaries are immediate family members or, in     some cases, business partners. These are the people who will benefit     from your continued good health.

    I am the beneficiary of one of your life policyholders who recently     passed away. What are the basic requirements when filing a death     claim?

    The initial requirements when filing a death claim are:

    • Insured’s Death Certificate duly signed by the Office of Local Civil       Registrar
    • Birth or Baptismal Certificate of the Insured
    • Original Life Insurance Policy Contract

    Immediately send the documents to our Head Office for processing.     You will also receive a call should we need additional claim documents     from you.

3. Maintaining My Policy

    Can I make a loan against my policy?

    Depending on your plan type, you may apply for a loan amounting to as     much as 80% of the earned cash value of your Policy in case of an     emergency.

    I was not able to pay my policy's premium due on its due date. Does it     mean my policy has already lapsed?

    Your policy has a 30-day grace period. You may still pay your premium     within 30 days from it due date.

    My policy has lapsed. What should I do?

    Send us a request to reinstate your policy by submitting an Application     for Reinstatement form. Upon payment of the outstanding premium,     interest & other charges, we will process the approval of your policy     reinstatement.

    I have a permanent life insurance policy from Generali. I have been     paying my premiums for several years but I can no longer continue     paying for it. What options do I have?

    We can still provide you the following protection options:

     Reduced Paid-Up Insurance – Use your policy's net cash value to     purchase a paid-up insurance plan as your original policy. However, the     amount of your new coverage will be smaller than that of your original     policy.

     Extended Term Insurance – Use your policy's net cash value to pay for a     term insurance plan with the same amount of coverage as your original     policy but with a shorter protection period.

     Policy Loan – You may take out a loan against the policy's cash value to     pay for your overdue premium. This will keep the original policy in force     for the full amount of coverage, as long as the cash value is sufficient to     pay for the due premiums. However, should the loan amount (inclusive     of interest charges) exceed the policy's cash value, the policy will lapse.

    I recently took out a loan from a bank and they required me to secure a     life insurance policy as part of the requirements. Can I assign my     Generali Pilipinas Life Insurance Policy for this purpose?

    Yes, if unencumbered, your policy may be assigned to the bank. You will     be required to submit a notarized Assignment of Life Insurance Policy     form which you can obtain from our Client Services Department (CSD) or     at any of our Sales offices.

    How do I inquire about the status of my policy?

    You may call our Client Services Hotline at (632) 884-8341 to 46 from     Monday to Friday between 8:00 AM to 5:30 PM.

4. Updating My Records

    I got married last year and moved to a new home. How do I update my     name and contact details in your record?

    Simply fill out a Policy Amendment form. For your convenience, we can     have it emailed to you upon request. You may also get a copy from our     Head Office or from your servicing agent. Prior to submission of the     form, please remember to attach a photo copy of your valid ID with     picture for verification purposes.

5. Paying My Premiums

    How do I pay the initial deposit for my life insurance application?

    You may pay the initial deposit for your peso and dollar denominated     policies over the counter at any BDO branch nationwide.

    A. Fill out the orange “Bills Payment Slip” and indicate the following:

        A.1 The Company's name (Generali Pilipinas Life Assurance               Company)
        A.2 Date
        A.3 Institution code
        A.4 Your policy reference number
                    Example of your policy reference number:
                    I – 000020015 -
                    I - The letter ''I'' represents the premium deposit used for                          the Initial Payment
                    000020015 - corresponds to the application number with the                          last digit indicated being a check digit identifier
                    1 or 2 - represents the currency identifier of the                          application  (1 for Peso or 2 for Dollar) which should be                          indicated in the box at the end of the Application Number                          sequence. This box should also be filled up in the                          Application Number sequence located at the upper right                          hand side of each of all succeeding pages of the                          Application Form.
        A.5 Your name

    B. Indicate the payment amount and currency on the lower half portion         of the “Bills Payment Slip”
    C. Proceed to the counter to make your payment.

        Important to note when filling up BDO Bills Payment Slip:
            Indicate the proper institution code as follows:
                        - 0048 for Peso payments
                        - 0080 for US Dollar payments
            Indicate the corresponding currency identifier
                        - 1 for Peso
                        - 2 for US Dollar payments

    When paying by check, please indicate Generali Pilipinas Life Assurance     Co., Inc. as the payee.

    How do I pay subsequent premiums?

    You may continue paying your peso or dollar denominated policies over     the counter at any BDO branch nationwide.

A. Fill out the orange “Bills Payment Slip” and indicate the following:
    A.1 The Company's name (Generali Pilipinas Life Assurance Company)
    A.2 Date
    A.3 Institution code
    A.4 Your policy reference number
          Example of your existing policy reference number:
          S – 12345678 – n – N
          S - The letter "S" indicated in the first position shows that the                payment is for an existing policy. This prefix will be used for all                Subsequent Payments made after approval of application and                issuance of the policy.
          12345678 - the numerics occupying the 2nd to 9th places in the                sequence, corresponds to the old policy number.
          n - the n occupying 10th place in the Policy Reference Number                sequence represents a numeric, which is a "check-digit" for the                old  8-digit policy number. This "check digit" helps                validate the policy reference number indicated on the Bills                Payment Slip by payers and further expedites payment                processing.
          N - the N occupying 11th place is a numeric Currency Identifier 1 or                2, where: 1 will signify "Peso" (PhP) 2 will signify "Dollar" (USD)
    A.5 Your name

B. Indicate the payment amount and currency on the lower half portion of     the“Bills Payment Slip”
C. Proceed to the counter to make your payment.

    Important to note when filling up BDO Bills Payment Slip:
            Indicate the proper institution code as follows:
                        - 0048 for Peso payments
                        - 0080 for US Dollar payments
            Indicate the corresponding currency identifier
                        - 1 for Peso
                        - 2 for US Dollar payments

    When paying by check, please indicate Generali Pilipinas Life          Assurance Co., Inc. as the payee.

    OTHER OPTIONS FOR PESO PAYMENTS (PESO POLICIES)

    1. SM Bills Payment Facility

        Simply present your Premium Notice to any SM Bills Payment counter         and fill out the BPS Validation Slip. Please indicate the company         name, your name and your Policy Reference Number as indicated on         the billing notice.

        * Company Name: Generali Pilipinas Life Assurance Company, Inc.

    2. BDO Auto Debit Facility

        For all BDO depositors, you may use the Auto-Debit Facility for         payment of a peso denominated policy. Simply visit your BDO branch         account and fill out the Authority to Debit Account Form. Upon due         date, your premium payment will be debited against your BDO         account.

    3. Credit Card Auto Charge Facility

        You may enroll your VISA, Mastercard, JCB and Amex credit cards with         Generali Pilipinas so your credit card account can be charged         automatically when your premiums fall due. Please call our Client         Services Hotline at (02) 884-8341 so we can walk you through the         credit card enrollment process.

    FOR DOLLAR PAYMENTS (DOLLAR POLICIES)

    TELEGRAPHIC TRANSFER OR WIRE TRANSFER

    For payments made outside of the Philippines, you may pay through     telegraphic or wire transfer from your bank account to Generali Pilipinas'     bank account using the following details:

    * Please note that applicable bank charges will be for the account of the     policyholder/client.

    REMINDER:

    Please send the payment details via fax to (+632) 884-8351 or (+632)     893-4195 and attention to “Finance Operations-Cashiering” to facilitate     Official Receipt issuance. Non-reporting of telegraphic transfer     remittance will result to non-issuance of Official Receipt and your     policy remains unpaid in our records.
